What form do I need to use to post a video of our school’s Christmas performance?

Christmas Concert’s are usually a public performance and as such do not require FOIP forms.
Schools should be cautioned that should a newspaper come to the school following the concert asking the school to identify children who performed – and if the child was not introduced during the play – then FP013(Updated to AF 180-C )should be sent home to the parents.  The reason you use FP 13 (Updated to AF 180-C ) is because most newspapers publish in paper and digital format and it can be copied and downloaded online.

Any public performances do not need FOIP forms.

I would like to post pictures of the Mother’s Day Tea to our school website, what forms do I need to have signed?

It depends who is in the picture and where it was taken.  If identifiable children are present consent should be FP013  (Updated to AF 180-C )Permanent Consent for Public Use of Student Images/Work for students – adults can give written consent.  No consent is required for unidentifiable children/adults in photos.

I have a video of a group of students working together to build a project. What forms do I need to have signed before I can post this on our school YouTube account?

If the students are identifiable – FP013 (Updated to AF 180-C ) Permanent Consent for Public Use of Student Images/Work  must be completed for each student – if you only see the backs of heads or the photos contain unidentifiable people – no form is required.
